Consent form for tattoo procedure

AFTER HOURS STUDIO

The artists at After Hours are self-employed, which means that any agreement regarding tattooing is made between you and the artist. We follow strict hygiene procedures and only use EU-approved ink to minimize the risk of complications. If you need follow-up, we ask that you contact the artist who performed your tattoo.

HEALTH INFORMATION

Getting a tattoo will always involve some risk of infection, allergic reactions and scarring. You are required to inform the tattoo artist of any health conditions that may affect the tattooing process. The following conditions are relevant:

-Immune system diseases

- Bleeding disorders

-Diabetes

-Allergy to dyes

-Pregnancy and breastfeeding

- Skin diseases (e.g. psoriasis, eczema, keloid formation, atopic dermatitis) - Medication use (e.g. blood thinners or immunosuppressants)

-Blood-borne diseases (e.g. hepatitis, HIV)

AFTERCARE

It is important that you follow the artist's advice on aftercare to achieve the best possible result and minimize the risk of infection. There is no guarantee of a perfect result, as every tattoo is a unique piece of work and each person will have an individual healing process. Areas of wear and tear such as hands, fingers, feet, knees and elbows will usually not heal optimally, and touch ups on these areas are done for a minimal fee. If you need a touch up, you can contact the artist once the tattoo has fully healed.

RESPONSIBILITY

A tattoo is a permanent decision, and therefore has no right of withdrawal. You are fully responsible for any consequences of getting a tattoo. You must be 18 years of age to get a tattoo with us. During the tattoo, you cannot be under the influence of alcohol or drugs that may impair your judgment. By consenting to the tattoo, you release the artist and studio from any liability for any complications related to the tattooing process.
